The aims of the District Nursing Service are to:
- Assist the safe transfer of patients from hospital to their home setting.
- Provide care to patients who are unable to independently manage their health care needs so preventing hospital or care home admission where possible.
- Support each person to obtain, regain or maintain their optimum state of health and independence and quality of life within the scope of any physical, psychological or social limitation.
This team can help you if you are unable to come to the surgery for your dressings, bloods, elderly screening check or flu injection and can help refer you onto other services such as Chiropody or Occupational Health Department where necessary.
