You can leave the Doctor's list at any time. This may be because you are
moving away. You do not have to tell us you are leaving, but it would be
helpful to us to know.

In very rare circumstances you may be removed from the list. This would
only occur when we feel that the trust that must be present between you and
the Doctors no longer exists. Any abuse of, or violence towards, the staff
or Doctors will lead to immediate removal.
