If you wish to be seen regularly, you will have to join the Practice list.

Once you are registered with the Practice you are free to see any Doctor you
choose. Please note that for continuity of care it is always to best to try
and see the same Doctor throughout a period of treatment to ensure
continuity of care.

If you are registering your baby then please bring along the form given to
your by the Registrar of Births.

If you are moving from another Doctor please bring your Medical Card. This
is a yellow/buff card issued by the Health Board where you last lived.  Please sign this card and hand it to one of the receptionists. Don't worry if you can't find your medical card, as we can give you a form as an alternative.

When you have completed the paperwork the Receptionist will offer you an
appointment for a brief health check with the Practice Nurse.
