Certificates/Sick Notes
When you are unfit to go to
work there are a variety of
certificates that are used to claim
sick pay.
If you are off sick for less than 7
days then you must sign yourself off
work by filling in a
self-certificate form supplied by
your employer. You do not need a
Doctor's certificate. These forms
are also available from our
reception.
For longer than 7 days come and see
a Doctor to be given a medical
certificate. Sometimes we can give
you a certificate without seeing
you, for example, if you have a
hospital letter about the illness,
otherwise you must be seen within 7
days of being off work. Hand in the
certificate to your employer.
Insurance
Certificates
If you are off work and have private
illness or accident insurance then
the insurance company will want
forms to be completed to tell them
about your medical problem. We are
happy to complete these forms but
there is a charge. Fill in your part
of the form, sign it and leave it
with the Receptionist. It will be
ready for collection in 5 working
days.
Medical
Examination
If you require an examination
carried out e.g. PSV, HGV, DVLA,
adoption, pre-employment, please
contact our Receptionist who will
advise you which of the GPs is able
to do this for you. She will then
direct you to speak to the GP during
his/her phone time to organise
between you when to attend to have
this carried out. There is a charge
for this service.
Solicitors'
and Insurance Company Reports
In some cases your
insurance company or solicitor will
write to your Doctor and ask for
further medical details about you.
We will only do this if you have
given us written permission for your
complete medical record to be
released.
Be Careful - sometimes you
will be asked to give permission for
your complete medical record to be
released or copied and you need to
be sure that you are happy to do
this.
