District Nurses, Health Visitors,
Midwives and Social Workers all
support
our Practice patients.
District
Nurses
The aims of the District Nursing
Service are to:
- Assist the
safe transfer of patients from
hospital to their home
setting.
- Provide care
to patients who are unable to
independently manage
their health care needs so
preventing hospital or care home
admission where possible.
- Support each
person to obtain, regain or
maintain their optimum
state of health and independence
and quality of life within the
scope of any physical,
psychological or social
limitation.
This team can help
you if you are unable to come to the
surgery for your dressings, bloods,
elderly screening check or flu
injection and can help refer you
onto other services such as
Chiropody or Occupational Health
Department where necessary.

You will be asked to leave your
message on their answer phone, which
they
will check on their return into
their office. Please speak clearly
into this leaving your contact name
and telephone number for them to
help you.

Health
Visitors
Their role is to help support and
advise families with children under
five years old and elderly patients.
They run regular clinics, provide
childhood immunisations and also
visit patients at home where
necessary.
- Tuesday
afternoon pre-school clinic 2 -
4 pm
- Thursday
afternoon baby clinic 2 - 4 pm
